Go get some blank decal paper (Testor's makes clear and white) and run it through your printer (inkjet, not sure about laser). Once dry you hit with some fixative (Future works or the can of stuff that comes with the Testor's kits works too), usually two coats or so and let dry. Then you apply like any other water slide decal.
As for making them you need to either find the artwork online or make it yourself in Paint, Photoshop, Illustrator, etc... Word can work too if you only need type, just find the appropriate font (there's billions online). Scaling them properly can be a bit of a PITA if you don't have Photoshop or an equivalent though.
