The book has finally appeared on the US Amazon. It says the availability date is 1 Nov 2008, but they have been wrong before:

Unfortunately I was a victim of card fraud... so I had to cancel my card. As soon as the new one arrives (hopefully this week!) I'll be sure to preorder!
